Returning Student Registration Form

Each year, Christian LIFE Academy requires returning students to complete Returning Student Registration Information (see below); this information is important so that we can maintain updated contact information for our students. The forms should be completed promptly when the notification is sent out.
Please complete the form online or download the Pdf version and submit a hard copy.
For your student to be registered for the next school year, you must complete the registration by June 24, 2022.
